Team Primary Care is an interprofessional initiative of the Foundation for Advancing Family Medicine funded by Employment and Social Development Canada. It is co-led by the College of Family Physicians of Canada and the Canadian Health Workforce Network, in partnership with over 100 health professional and educational organizations across Canada.

  • The Foundation for Advancing Family Medicine (FAFM) was established in 1994 to better serve the future of family medicine. The Foundation is dedicated to advancing the discipline of family medicine through research and education initiatives.

Project Leadership

  • The College of Family Physicians of Canada (CFPC) is the professional organization that represents more than 42,000 members across the country. The College establishes the standards for and accredits postgraduate family medicine training in Canada's 17 medical schools. It reviews and certifies continuing professional development programs and materials that enable family physicians to meet certification and licensing requirements.

  • The Canadian Health Workforce Network is a knowledge exchange network of researchers, decision-makers and other knowledge users with expertise in health workforce planning, policy and management.

  • ACCES Employment provides a wide range of effective job search programs and services. We have seven locations throughout the Greater Toronto Area to serve you and we provide the convenience of online services that are available to jobseekers locally, nationally and internationally for approved pre-arrival immigrants. Our virtual service offerings include information sessions, webinars and online programs, as well as VERA, our Virtual Employment and Resource Attendant who is available at any time to answer your questions.

  • Founded in 1945, first as the Canadian Conference of Pharmaceutical Faculties (CCPF), the Association of Faculties of Pharmacy of Canada (AFPC) is a non-profit organization and is committed to advancing pharmacy education and research. AFPC is the national voice for academic pharmacy, providing leadership and resources for faculty members across Canada. Almost 1,300 new pharmacists graduate from the ten universities each year and about 600 students are enrolled in graduate programs.

  • The Black Physicians of Canada aim to build a community of Black physicians and physicians-in-training as well as creating social networks and learning together. We will focus on providing mentorship for Black physicians and physicians in training, as well as advocacy, representation, guidance, support, encouragement, resources and advice.

  • CAEP provides continuing medical education, advocates on behalf of emergency physicians and their patients, supports research and strengthens the EM community. In co-operation with other specialties and committees, CAEP also plays a vital role in the development of national standards and clinical guidelines.

  • The Canadian Association of General Surgeons (CAGS) is Canada’s only national organization representing the interests of General Surgeons. Our Mission is to empower general surgeons to improve patients’ lives through continued advocacy, education, and research. We advocate on behalf of academic, community, rural and remote General Surgeons on issues related health human resources, transition of practice, pay equity, and the required access to resources to provide care for patients, and for quality of care for surgical patients.

  • CFPNA is a volunteer Nursing Association that provides a national voice for nurses in primary care through leadership, mentorship and fostering expertise.

  • The Canadian Society for Medical Laboratory Science (CSMLS) is the national certifying body for medical laboratory technologists and medical laboratory assistants, and the national professional society for Canada's medical laboratory professionals. We are a not-for-profit organization that is funded entirely by membership dues and revenues from goods and services. We do not receive operational funding from governments or other organizations.

  • CaSPR is a professional not-for-profit society that consists of individuals who are involved with physician recruitment from across Canada. CaSPR was initially formed as a Regional Chapter of the Association for Advancing Physician and Provider Recruitment and maintains a liaison relationship with theparent organization.

  • The Canadian Association of Physicians with Disabilities is a not-for-profit, social networking and support organization for physicians in Canada hoping to connect with one another.

  • The Canadian Association of Social Workers (CASW) was founded in 1926 to monitor employment conditions and to establish standards of practice within the profession, the CASW has evolved into a national voice. The CASW Federation is comprised of 10 provincial and territorial partner organizations.

  • ​​​The Canadian Interprofessional Health Collaborative (CIHC) has been working for more than a decade to strengthen the knowledge base and ​practice for collaborative,patient-centred practice.

  • The CMHA is a federated charity, which means we are a collective of organizations bound together by a brand and mission. Together, we identify and respond to Canada’s most pressing mental health priorities. At the national level, we push for nationwide system and policy change. At the community level, millions of people in Canada rely on CMHA’s extensive grassroots presence. 

  • CPA recognizes a society where understanding of diverse human needs, behaviours and aspirations drive legislation, policies and programs for individuals, organizations and communities. CPA is committed to advancing research, knowledge and the application of psychology in the service of society through advocacy, support and collaboration.

College of Pharmacy

  • ​​The PharmD/MHA program combines a professional, patient-oriented qualification with a master’s degree that provides the systems level thinking needed by health care leaders. This program builds on the success of the long-standing JD/MHA which has proven to bring diverse perspectives that enrich the classroom experience of all students.

  • ​​Dalhousie Family Medicine is committed to advancing the discipline of family medicine through research and education in primary health care, teaching to family medicine residents and to Dalhousie medical students and demonstrating our commitment to lifelong learning and the dissemination of knowledge to Maritime family physicians.

  • The Equity in Health Systems (EqHS) Lab was established in September 2021 by Dr. Jerry Maniate.. Drawing together leaders, influencers and learners from diverse backgrounds has facilitated the creation of a unique community and social network that is based on shared principles and approach.

  • ​​HMC Connections was founded in 1978 by volunteers from a number of local cultural groups who wanted to create a welcoming community for newcomers and refugees.

  • As Indigenous physicians and students who are diversely rooted in our traditional teachings and our respective communities, IPAC members embrace and commit to collaboratively use our skills, abilities and experiences to advance the health of our nations, communities, families and individuals.

  • The Indigenous Primary Health Care Council is focused on supporting the advancement and evolution of Indigenous health care service provision and planning throughout Ontario.

  • Part of the UHN Research family, TIER plays a critical role in improving the quality of education and overall competency of learners, while helping to ensure health care providers across all disciplines are well-poised to respond to emerging technologies and medical breakthroughs.

  • The Midwives Association of British Columbia (MABC) is the professional association for midwives in BC and is registered under the Societies Act.
